This beautiful, delicately carved mother of pearl hand c. mid-late 1800s is what is known as a "mano cornuto" or horned hand amulet.
The mano cornuto has historically been worn as an amulet of protection against the evil eye, and more recently (in the last 50 years or so) has become popular in the music scene to mean "rock on." Students of the University of Texas, the Texas Longhorns, have also adopted it as their hand sign.
The setting is 9k gold.
